Overweight participants lose half their body weight through diet and exercise under the guidance of a trainer. After losing the weight, many of the participants choose to undergo plastic surgery to complete their Extreme Makeover with dramatic and medically necessary skin removal surgery. In each of the episodes, the participants undergo a transformation of not only their bodies, but of who they are as individuals.
